- Use of the form above is ONLY for voters on the permanent absentee ballot list who wish to:
- Make a change as to where their ballots are mailed for one election, or
- Change the political party primary ballot they receive for one or all elections.
- Use of the form above is ONLY for voters on the permanent absentee ballot list who wish to:
- Request to Remove Name from Permanent Absentee Voter List
- Use of the form above is for those who wish to be removed from the permanent absentee voter list. Once removed from this list, you will only be mailed a ballot if you re-request to be mailed a ballot. Otherwise, you may vote in-person either early or at your polling location on election day.
